Extension of land lease

Applicants for the extension of land lease are required to bring along the following to be submitted at the "Unit pelupusan" of the Petaling District Land office:

1. OFFICIAL SEARCH
(HS(D) @ PN - STATE LAND TITLE REGISTRATION OFFICE, SHAH ALAM)
(*If applicant does not have/hold the original title)

2. ORIGINAL LAND TITLE

3. TWO PIECES OF THE LOCATION PLAN VERIFIED BY A PETALING DISTRICT OFFICE SURVEYOR
(Please purchase at Bahagian Pelan [Unit Teknikal] of the Petaling Land/District Office)

4. COPY OF IDENTIFICATION CARD OF APPLICANT

5. COPY OF RECENT QUIT RENT RECEIPT

6. COPY OF RECENT ASSESSMENT TAX RECEIPT

7. PERMISSION LETTER FROM THE BANK/INDIVIDUAL
(*If title has charge/caveat)

8. FEE OF RM 100.00 (1 LOT) FOR APPLICATION TO EXTRADITE LAND (FORM 12A)
(*Must be duly signed by a lawyer (kindly include IC number of lawyer and lawyer's number) or land administrator [pentadbir tanah] under the "akuan saksi" space/section.)

9. APPLICATION FEE OF RM50.00 FOR RESIDENTIAL BUILDING

10. APPLICATION FORM TO EXTRADITE & RE-APPLY FOR LAND (INDIVIDUAL)
(*Forms are obtainable at the Petaling Land/District Offict OR the Bukit Gasing Assemblyman's office @ MBPJ Hockey Stadium, Jalan Selangor, 46050, PJ, Selangor: Tuesdays (recommencing only on Tues 31 Jan 2012) 8.30pm-10.30pm)
